A District Nursing Specialist Practitioner Qualification is essential for this post. In addition, you must hold a UK driving licence and have use of a car.

Main duties of the job
We work autonomously to ensure safe, highquality care within the community nursing team, taking responsibility for clinical activity and daily service coordination. I assess and manage care for patients with complex needs, considering health, social, and environmental factors. I lead effective triage and caseload allocation, working closely with the MDT to maintain continuity of care. I provide clinical and managerial supervision to registered staff, HCAs, and students, supporting competence, development, and reflective practice. I collaborate with professionals and partner agencies to promote selfmanagement, appropriate delegation, and improved patient outcomes. I deliver direct patient care and contribute to specialist interventions while acting as a clinical role model. I manage daytoday team operations, deputise for the Senior District Nurse, support costefficient service delivery, and help maintain a positive workplace where highquality practice, staff wellbeing, and learning are prioritised
