Job summary

Do you have a passion or desire to work with people in the community? We are seeking an enthusiastic and motivated Band 5 Occupational Therapist to join our Community Therapy team. In this role, you'll provide high quality, person centred care for adults in the community.

This is an excellent opportunity for a compassionate, adaptable therapist who is keen to develop their clinical skills, work collaboratively, and make a tangible difference to people's lives.

Main duties of the job

Deliver high-quality, person-centred rehabilitation for adults in the community.
Assess, plan and implement tailored treatment and rehabilitation programmes.
Work collaboratively within an interdisciplinary team across community setting.
Communicate effectively with patients, families and carers, supporting goal-setting.
To manage a caseload, using evidence based/person centred principles to assess, plan, implement and evaluate therapy interventions.
Accurately record assessments, monitor progress and adjust care plans as needed.
Support service efficiency, audit activity and performance monitoring. Supervise and develop rehabilitation assistants as required.
Maintain professional standards, mandatory training and uphold Trust values, safeguarding and governance.
