Job Summary
We would like to offer an opportunity for a motivated and passionate individual to join our Memory Assessment Service as a Band 6 RMN or MHP. This is a full time role but we will consider requests to work alternative hours or varied working patterns in line with our flexible working policy and to meet our service requirements.
Your main responsibility will be to undertake initial assessments and reviews of patients referred to the memory clinic, therefore it is essential that you have an excellent knowledge of older peoples mental health and cognitive difficulties. You will also have experience of working within older person's mental health including people with organic difficulties. We offer a comprehensive induction.
We are a service which is committed to delivering high quality and compassionate care for people aged over 65 years old with a suspected diagnosis of dementia. The service provides identification of dementia or cognitive impairment, as well as differential diagnosis. These assessments mainly take place in an outpatient setting but can also include home visits. We also offer virtual assessments on occasions.

Main duties of the job.
- You will be responsible for carrying out specialist mental health assessments for individuals with memory impairment, imparting diagnosis, offering advice and signposting and commencing treatment in collaboration with the MDT.
- You will undertake the role of Memory Service Duty Clinician on a rota basis.
- You will take part in our triage process where we screen referrals for suitability to our service and offer signposting, advice or redirection where appropriate.
- You will attend and contribute to the weekly MDT where your assessments will be discussed with a designated medical supervisor and other professionals.
- The Memory Service works across Leicester, Leicestershire and Rutland and we hold outpatient clinics at various sites. Our two main hospital sites are The Bennion Centre (Glenfield General Hospital) and The Neville Centre (Leicester General Hospital). We also hold satellite clinics across the county and complete home visits when necessary.
