We have an exciting opportunity for a motivated qualified band 6 Mental Health Nurse, Occupational Therapist or Social Worker to join our team as a RECONNECT Registered Practitioner within the Directorate of Mental Health.
The Reconnect registered Practitioner will take a lead role in the ongoing development of the RECONNECT Service across Leicester, Leicestershire, and Rutland, The purpose of this role is to deliver care after custody to individuals with a health need, released after remand or on completion of sentence from Leicester, Leicestershire, and Rutland (LLR) Prisons such as HMP Leicester, HMP Stocken and HMP Fosse Way; or for individuals leaving an immigration removal centre. The post holder will have a keen interest in health and justice and a passion for supporting people as they are released from prison and will work as part of a team and liaise closely with Criminal Justice Partners to work with individuals who have been referred.
The Successful candidate will be providing screening and assessment to build a personal plan for the service user to enable a smooth transition into community life.
Main duties of the job
To perform comprehensive assessments of needs and risk assessments with service users. To formulate support plans collaboratively with service users and their carers/relatives. To involve service users and carers / relatives in the assessment, planning, delivery and evaluation of support.
To oversee the support delivery provided to service users by RECONNECT engagement workers and peer support workers. To work collaboratively with key stakeholders and professionals within the criminal justice setting including within the prison and probation service to support the development and maintenance of effective referral pathways.
