We have an exciting opportunity for a Band 6 Community Mental Health Nurse (Older People) to join our team within the Directorate of Adult Mental Health. The role which involves managing a caseload of older adults with mental health needs, acting as a care coordinator, and providing specialist, evidence-based care. Key responsibilities include conducting assessments, developing care plans, liaising with other professionals, and supporting service users and their carers in the community, while also supervising junior staff and potentially taking on leadership duties.

Main duties of the job
Core responsibilities: Care coordination: Manage a caseload, including initial assessments and the coordination of care plans for older adults and their carers. Clinical practice: Provide high-quality, specialist, and evidence-based mental health nursing care in a community setting. Assessment and planning: Conduct comprehensive assessments, formulate person-centered care plans, and assess risks. Multidisciplinary teamwork: Work with a multidisciplinary team, liaising with other professionals in primary and secondary care. Support and leadership: Provide professional advice and support to other agencies and less experienced staff, acting as a role model and resource. Duty and emergency response: Participate in the duty rota, which can include undertaking emergency assessments. Record keeping: Maintain accurate and up-to-date records and reports
