Job summary
A rare and excellent opportunity has arisen for a Band 5 Occupational Therapist to join an established, motivated and forward thinking Adult Learning Disabilities Occupational Therapy Service.This post is for 30 hours a week (0.8 WTE).
An Occupational Therapist is required to work within the Community Learning Disability Team, providing proactive support to individuals with Learning Disabilities and additional health needs.
This post will offer you the opportunity to work within a multidisciplinary team approach, in a variety of settings within the community, providing specific healthcare interventions.
Essential for this post is effective communication and organizational skills; we would welcome applications from candidates who demonstrate confidence, self-awareness and an enthusiasm to work with this client group.

Main duties of the job
- To carry out the Occupational Therapy Process (information gathering, assessment, treatment planning, intervention, evaluation and discharge planning) as appropriate working with a person centred approach.
- To work flexibly within the multidisciplinary team providing a holistic approach to meet the needs of the patient and service.
- To offer education, advice and training to informal and formal carers to best support the patients safety.
- Ensure patient information is collated, monitored and fully accurate, maintaining complete and contemporaneous records in line with HCPC/professional bodies and Trust standards.
- The post holder will work closely with multidisciplinary colleagues, regarding clinical work, and with other professionals from within the appropriate MDT and outside agencies as appropriate.
- Knowledge of professional standards of practice, code of professional conduct and ethics.
- Record keeping and report writing skills.
- Previous experience of working with the patient group.
