Job summary
We are looking for an experienced and enthusiastic Physiotherapist to join our well-developed, long-standing Community Therapy service. The service offers an urgent community response to ensure patients can be supported to stay at home to avoid hospital admission, as well as support those returning home from a hospital setting for ongoing rehabilitation to meet their full potential in their own environment. Alongside our Urgent Community Response offer we also offer community therapy to those patients with non-urgent routine therapy needs as well as therapy input into High dependency Discharge to assess beds (D2A) for patient who may have cognitive impairment, known delirium or a non-acute mental health diagnosis that require 24 hour care and 1:1 support, to provide recovery, reablement and rehabilitation, with an aim for discharge home and determine long term care needs. Successful candidates with be expected to work autonomously providing assessment and treatment/rehabilitation in peoples own homes. We work closely with our other MDT colleagues, holding daily MDTs to support our patients in a collaborative way. Applicants must hold a valid UK driving license, always have access to a vehicle for work purposes on duty and confident to drive in all driving conditions for this community post.
