Job summary
We have an exciting opportunity for an Occupational Therapist, Physiotherapist or Speech and Language Therapist to join the newly commissioned Leicestershire Partnership Trust (LPT) Acquired Brain Injury Case management service. LPT has been running the service since January 2024 and we are now looking to expand our clinical team. We provide case management services to patients with acquired brain injuries or strokes who require slow stream rehabilitation, overseeing their rehabilitation and ensuring they get the best outcomes possible. This is a rewarding job, working closely with the people affected by ABI, we work closely with the Integrated Care Board to agree funding for these packages of rehabilitation and care, and with wider community services at the point of discharge from the pathway. We are looking for someone passionate about working with acquired brain injury and stroke who is committed to improving the coordination of care and outcomes for these patients. We are looking for someone with experience of working with neurological conditions, acquired brain injury and stroke with an understanding of the pathways for care for these conditions. Previous case management experience would be helpful but is not essential as you will be supported by the band 7 ABI case manager to develop these skills. Main duties of the job
To work as part of a dynamic team to ensure safe, effective, and high-quality care pathways are in place for all patients within the service, including case management responsibility for the patients on the ABI pathway. To work with acute health services and slow stream rehabilitation placement providers to facilitate/coordinate personalised care and rehabilitation for patients whilst on the ABI slow stream pathway. To work closely with the CINSS/ABI Clinical Service Leads to organise, develop, and maintain an efficient and effective ABI funded pathway service to LLR patients in the post-acute community setting. To have responsibility for monitoring the implementation of ABI clinical guidelines and personalisation of rehabilitation programs within provider services. To work as a specialist clinician to utilise, develop and enhance Occupational Therapy/Physiotherapy/Speech and Language Therapy clinical and professional skills within the Service to deliver high standards of practice. To have clinical responsibility for a designated specialised caseload across the service. To work flexibly within the team, which may include supporting CINSS clinical pathways dependent on service needs.
