A new and exciting job opportunity has arisen within the Adult Crisis Resolution and Home Treatment Team based at the Bradgate Mental Health Unit.

The Care Navigator will support/advise patients and their carers/family members open to the Crisis Team, who are deemed as not eligible for social care needs but have been identified by health/social care colleagues as needing ongoing support in social stresses such as domestic/housing issues/benefit advice/financial issues.

The post holder will work closely with providers of services and assist the patients they are working with in sourcing a solution to their care/support needs, will need to have a very good understanding of the types of provisions available in Leicester, Leicestershire and Rutland and be able to provide clear advice.

The aim of the Care Navigator role is to improve the journey for patients and their carers/family members as they navigate through Crisis, to improve co-ordination and access to support services for patients, with a view to improving their wellbeing/independence/safety at home.

Please note you will be expected to complete home treatment and visit patients in their own home therefore driving is essential to this job role as public transport will not be suitable.

Main duties of the job

This is what we need you to do:

Provide information/support/advice to patients and their carers/family members about services available to promote health and wellbeing and maintain independence within communities. Facilitate access to services available in the community. Develop knowledge of local support services to enable the patient to access a range of services to meet their needs. Ensure patients are aware of support in their local community, to make best use of resources. Actively participate in multi-disciplinary team meetings on a regular basis, to identify patients that may need support. Liaise with a range of multi-disciplinary professionals who are involved in a persons care, ensuring a smooth and co-ordinated approach, especially where multiple agencies are involved.

The role will require the post holder to carry out daily updates/recording onto patients electronic records (SystmOne) You will also cover the role and responsibilities of Acute Recovery Worker, providing psychosocial interventions and coping strategies to our patients.

The ideal candidate will have the following experience:

Significant practical experience of working with patients/ service user groups giving rise to a sound understanding of the services available, work practices and processes relevant to the role. Ability to engage with service users and partner agencies within the relevant codes of practice values (dignity, respect etc).
