Job summary
An exciting opportunity has become available to work within the Community Enhanced rehabilitation Team as a Band 6 nurse. Here at CERT we have cohesive and fun team who make coming to work a pleasure whilst ensuring the highest standards of care are at the heart of everything we do.
We are looking for an enthusiastic and dynamic team player, to join this growing and developing team. CERT is a psychology led, community mental health rehabilitation team within the Enhanced Rehabilitation and Recovery Pathway. Comprising a multidisciplinary team; CERT provides mental health rehabilitation focused interventions in the community for adults for adults with severe and enduring mental health difficulties. We work within a recovery approach, undertaking collaborative interventions with service users, families and wider networks in the community. CERT offers service users and carers a greater choice over their care and recovery, ensuring intervention are deliver in the last restrictive setting. The team will be able to offer you on-going CPD opportunities, reflective practice space within a fun and supportive team environment.

Main duties of the job
This post will include assessing new people referred to our service and working with an allocated caseload of existing service users providing specialised, enhanced rehabilitation and recovery interventions and liaising with other professionals to coordinate their care. You will need to demonstrate effective assessment, planning, monitoring and and evaluation of care given to service users. You will need to be able to safely administer medication in a community setting, engage in medicines management and physical health monitoring. You will undertake the role of lead clinician for service users who are under a Community Treatment Order.
You will need to demonstrate excellent communication skills and be able to provide clear information around health promotion and psychoeducation to people who use our services as well as providing educational input and supervision to nursing students and junior colleagues. You will participate in effective clinical leadership of the team.
You will need to be able to work flexibly and collaboratively with a multidisciplinary team and other agencies. You will need to be able to assess risk dynamically and work to safeguard our clients at all times.
