Job summary
The LDA Collaborative is committed to improving the lives of children, young people and adults with a learning disability and autism. This is an all-age post and will work as part of an integrated team working across Leicester, Leicestershire and Rutland.
We are looking for a Registered professional with membership of a relevant professional body, who is looking for an opportunity to develop their skills at B6 level.
You will be well supported by a friendly, experienced team, working across LLR systems. This post will involve undertaking placement reviews, CPA reviews and MHA Section 117 meetings.
You will oversee the effective case management and assure the quality of individual placements for adults and children who require specialist inpatient mental health or learning disability services/placements.
You will be required to establish and maintain effective links and good communication with all specialist mental health and learning disability service providers (NHS and independent sector).
You will need to ensure patients in specialist mental health/learning disability services receive high quality and clinically appropriate treatment and that this is in accordance with identified individual care pathways and in the least restrictive setting.
You will lead on the commissioning of individual packages of care in the community and monitoring of assurance outcomes, working to ensure individuals are safe and packages of care meet their needs.
Main duties of the job
- Undertake placement reviews, CPA reviews and MHA Section 117 meetings
- Oversee the effective case management of individual placements for adult and children service users who require specialist inpatient mental health or learning disability services/placements.
- Establish and maintain effective links and good communication with all specialist mental health and learning disability service providers (NHS and independent sector) who provide services to individuals from Leicester, Leicestershire and Rutland.
- Work effectively and collaboratively ensuring patients in specialist mental health/learning disability services receive high quality and clinically appropriate treatment and that this is in accordance with identified individual care pathways and in the least restrictive setting.
- Understanding of admission avoidance processes, ability to work in a multi-agency setting to identify appropriate joint solutions to prevent hospital admissions.
- Organise, lead and chair multi-disciplinary meetings, showing compassionate leadership and the ability to manage different perspectives whilst achieving good outcomes for service users.
- Attend LDA Collaborative Discharge Planning meetings to discuss discharge plans for all cases
- Ensure proactive action planning for patient discharges well in advance to meet expected discharge dates
- Act as Chair for patient Care (Education) and Treatment Reviews, as appropriate, and follow up actions from these reviews
