Job summary

Diana Children's Continuing Care Team provide respite/healthcare support to children/young people with complex healthcare needs, including life limiting/life threatening conditions in a variety of settings.

Primary role of the healthcare worker is to provide support in the home; however, you may be required to work in additional settings such as in schools, Child and Family Support Service, Phlebotomy, and children outpatient clinics.To provide effective, evidence based general and clinically specific care for patients in the community, under the indirect or remote supervision of registered practitioners.

Main duties of the job

  • To work as part of a nurse led service caring for children/young people with life limiting illnesses and/or technological support under the supervision of registered nurses.
  • Work in close collaboration with parents/carers to provide care to children and young people in accordance with the individual care plan to meet nursing and therapy needs.
  • Contribute to the assessment of childrens and young peoples health and wellbeing alongside qualified staff.
  • Monitor and evaluate childrens young peoples health and care management.
  • Take the lead role alongside qualified nurse colleagues caring for more complex children and young people
