The service is a key part of both health & social care strategies which are aimed at supporting those that need it to be independent at home/school, live at home with support, be treated in their own homes rather than in hospital settings & be discharged from hospital quickly when medically fit.

The service also supports hospital/care admission avoidance & intensive community support/rehabilitation when needed.

The position will be based at the Equipment Providers Depot in Glen Parva, however this service operates an office and home working approach. The post holder may need to travel to other health or social care sites across LLR.
