Our community mental health offer is undergoing a major transformation and the South Leicestershire Community Mental Health Team are searching for people with a range of skills and a passion to deliver great mental health services.

We are also looking for people who can develop relationships and work in an integrated way with a range of practitioners from outside of the NHS including; primary care, social care and the voluntary-community sector.

Main duties of the job
We are looking for enthusiastic, driven, and positive individuals with a commitment to working with recovery principles and interventions to join our busy team. The team environment is transformative and very supportive, and we always help each other out.
You will need to have experience of working with mental illness in either a community or inpatient setting, and the ability to manage your own diary and the demands of a caseload. You will be working as part of a multi-disciplinary team, carrying out duties autonomously and using your own initiative.
You will work as care co-ordinator for patients with a variety of mental health problems, providing high quality care in a community setting, promoting and ensuring recovery focused care and social inclusion. Working in partnership with services to aid the recovery of patients, both within LPT and the voluntary sector.
The successful candidate will be involved in and chair regular Multi-Disciplinary Team (MDT) meetings. These meetings allow you to use your clinical knowledge and expertise to support your colleagues. You will have opportunity to use your clinical assessment skills for new patients referred to the service, which will be discussed in the MDT for a team decision on the outcome.
You will be given a robust induction and will receive regular planned supervision, as well as ad hoc support when needed. You will be supported in developing your knowledge and skills with relevant training opportunities.
