We are excited to invite applications for a fixed term position of Team Manager within our mental health for older people speciality services. This is rare opportunity for a compassionate leader, with drive and a passion in MHSOP service delivery, to lead and manage The Memory Assessment Service, unscheduled Care team and In Reach team.
Within this role you will have responsibility for the development and operational management of Community Mental Health Services for Older People (MHSOP) within the locality, supporting the implementation of change to deliver strategies for improving service delivery within the service specification.
Developing close working relationships with Clinical Commissioning Groups, GPs, Local Authorities, Voluntary Agencies, Local Strategic Partnerships and the Independent Sector to develop pathways of care to meet the health needs of the population to optimise care closer to home and reduce dependency on secondary care.

Main duties of the job
The successful candidate will provide leadership to the service and to work collaboratively with colleagues across both professional and organisational boundaries to create an environment in which excellence in clinical care will flourish, working in partnership with Service Manager and other Community Managers to implement integrated care pathways within MHSOP.
The post holder will be responsible for effective and efficient financial and resource management to maximise the level and quality of services within budget allocation to achieve financial balance within services working closely with the Finance team.
Working with Head of Service and Community Team Leaders to implement Integrated Care Pathways within the Division, to initiate and be influential in the creation of new models of care and to promote the development of staff to achieve this.
The post holder will develop appropriate partnerships and joint working with local statutory and non-statutory agencies including patient and carer groups to optimise opportunity and support for patients. In collaboration with Service Manager, ensuring the service is planned, monitored, delivered and evaluated in accordance with evidence based practice, agreed standards and in line with Divisional priorities.
