Job summary

To support flow within the system, Leicestershire Partnership Trust has opened a new ward at Coalville Community Hospital. This is a discharge to assess ward with the aim of improving outcomes for our patients to enable them to be discharged home with appropriate levels of support in place.

This is an exciting opportunity where you will gain experience of establishing a new ward and developing new ways of working within a supportive multi-disciplinary team.

We are looking for candidates who have have a passion for physical rehabilitation of the older adult. You will possess excellent communication and problem-solving skills and have a positive approach to MDT working. There is the opportunity to be involved in establishing new ways of working and measuring outcomes whilst being supported by a Band 7 OT and Physio on site.

LPT offers a comprehensive mandatory training programme and the successful candidate will also be supported to undertake regular clinical supervision, participate in CPD activities and develop any specific areas of interest. There are opportunities to undertake further research or support Quality Improvement projects.

Main duties of the job

To provide patient centred, goal directed rehabilitation and effectively undertake discharge planning.

To work closely with the team of Physiotherapists, Occupational Therapists, Technical Instructors and the wider MDT.

To work with the MDT to maintain high standards of care.
