We are the Community Enhanced Rehabilitation Team (CERT), who assist service users in the community with their Mental Health recovery to promote independence and autonomy. Covering Leicester, Leicestershire and Rutland visiting service users in their homes, providing a holistic approach to Mental Health rehabilitation.

We are looking for an enthusiastic and motivated Health Care Support Worker, with experience in mental health care to join our friendly and supportive team.

We are a psychology led service, using psychological formulation to gain a bio-psycho-social understanding of individuals to aid their recovery and work towards their rehabilitation goals.

We currently work from home providing you with IT equipment to enable you to do this, however we do have work space available to us, should we need to meet in person.

We care about staff wellbeing and offer a supportive team environment and frequently meet of outdoor meetings to enhance our wellbeing.

Main duties of the job

The successful candidate will be working alongside Nurses and Occupational Therapists and working independently, visiting service users in their homes to deliver planned rehab and goal-based interventions.Supporting service users to maintain their health and wellbeing.

The post holder will keep clear and accurate records of your interventions with service users using the patient electronic records. Escalating any concerns regarding service users in a timely manner.

You will contribute to planning and implementing rehab based interventions and participating in Multi Disciplinary team discussions regarding the service users you visit. The successful candidate will alsobe trained in Decider Skills and supported to deliver a simplified combination of CBT and DBT techniques. Supporting other colleagues and learners, contributing to the development of the team.
