Job summary

We are currently looking for full-time nurse who is passionate about palliative and end-of-life care.

We are the Integrated Community Specialist Palliative Care Team (ICSPCT), who are passionate about caring for palliative patients from early diagnosis to end-of-life care.

We believe that we only have one chance to get it right.

The team includes a service lead, band 7 team leads, band 6 specialist nurses, senior health care assistants, and dedicated administrators.

The team delivers care to patients in their homes across Leicester, Leicestershire, and Rutland.

We are supported by two community consultants from LOROS. The ICSPCT is an aligned service from Leicestershire Partnership Trust and LOROS.

Main duties of the job

To work as part of a multi-disciplinary team and autonomously as a specialist palliative care nurse within a locality. To manage patients with complex health needs and support families and carers in the community.

To provide training and education both formally and informally for the development of professionals and communities involved in the provision of palliative and end-of-life care.

To demonstrate the organisational values and behaviors in everything you do in the work environment.

You will be supported with Continuing Professional Development and should be willing to work towards the Non-Medical Prescribing qualification.

  • Registered Nurse/Midwife (for Health Visitors only) Level 1. Degree or equivalent. Post-registration qualifications. Evidence of continuing professional development within the last 12 months. Hold an NMC mentoring qualification.
  • Leadership or management qualification or willingness to work towards. Non-medical prescribing qualification or willing to work towards

Experience

Essential

  • Experience in working with palliative care patients. Ability to assess patients with complex needs. Able to work autonomously. Good communication skills. To give support and advice to the wider MDT. To liaise with GP surgeries and attend Gold Stand Framework meetings
