Job summary

Clinical lead of transitions in CAMHS Eating disorders Team

The transition nurse role is a brand new role to our team, developed with support from mental health investment schemes. It is an exciting opportunity to develop a pathway to ensure smooth transition between CAMHS and adult services to support alignment to the East Midlands Transition Standards for eating disorders.

The post will focus on ensuring smooth transitions for young people from children to adult services, including the adult eating disorder service and other adult care providers.

The successful candidate will be a named point of contact for young people, clinicians, and carers. The aim is to improve the experience of transitions for young people and help them to feel supported and safe at an often difficult and vulnerable time.

Applicants must be qualified mental health nurses with specific experience of eating disorder work and/ or experience of working in CAMHS. Ideally the applicant will also have post qualification training and experience of delivery of NICE concordant psychological intervention for children and young people with an eating disorder or associated health conditions.

The successful candidate will be expected to provide evidence based intervention for children and young people with eating disorders.

Main duties of the job

The successful candidate will support further enhancement of links with colleagues in adult services with a view to minimise distress and uncertainty for CYPs during transitions.

You will support development of the transition pathway and ensure concordance with the East Midlands Transition Standards and NICE guidance.

Work alongside our experienced and diverse MDT to provide tailored support in transitions and ensure appropriate post 18 care is being sought/ accessed.

Liaise with partners and key stakeholders to ensure the care being delivered in transitions is of high quality and tailored to the needs of our diverse population in LLR.

Support young people with diagnoses including but exclusive to: binge eating disorder, anorexia nervosa, bulimia nervosa, ARFID and other common comorbidities such as ASD, ADHD and mood related difficulties.

To have lead responsibility for transition and the continual development of an appropriate transition pathway.

To support families and young people by assessing their individual support needs including supporting families with complex systemic presentations in partnership with their lead professional and the wider MDT.
