We are currently recruiting for our Looked after Children's nursing team, to support our Looked After Child offer and will consider full- time or part- time hours.
This is a great opportunity to support and assess the needs of vulnerable children across Leicester, Leicestershire and Rutland.
You will be working in our friendly and supportive nursing team caring for our children and young people within the three local authorities in conjunction with families and care givers, to maximise their health potential and address their health inequalities.
The role will necessitate travel across Leicester, Leicestershire and Rutland, and you will work across the county.
We assess the health of this group of children and young people, implementing any necessary care pathways.
We offer an induction period to ensure you feel competent to carry out the role, with ongoing training and support for your development
Main duties of the job
As the ideal candidate you should have experience of the needs of Looked After Children and an understanding of the service we provide.
You should have a strong desire to improve the outcomes of these children and young people and demonstrate your understanding of the importance of working collaboratively with our multidisciplinary colleagues to achieve this.
You must understand the importance of safeguarding children as the overall case holder you will attend and contribute to safeguarding meetings, case conferences, Looked After Children's reviews, pathway planning meetings, professional meetings and strategy meetings, supporting the delivery of service to meet their needs.
You will demonstrate excellent leadership skills. Contemporaneous record keeping is key for our service.
You should be able to work on your own initiative but be a strong, supportive team member, demonstrating a well organised and flexible approach. We need cover for our core hours of 8 am -5pm, therefore an ability to work flexibly and manage conflicting caseload/workload priorities is key to success within this role.

where LAC is aged 5 18 to determine if the carer is progressing the health
care plan and to support them where they have difficulties including liaison

To complete leaving health care summaries, effectively
ensuring seamless transition of care to adulthood ensuring that (For LLR LAC
living in LLR and externally placed LAC in LLR where the RHA is commissioned)
Identifies areas where care interventions are required
and delivers comprehensive interventions in a timely manner including sexual
health, emotional health, etc
Completes holistic health chronologies and provide a
written comprehensive report detailing the implications of the information
for the childs current and future health and wellbeing for meeting such as
strategy discussions/ LAC meetings/Safeguarding meetings/CSE strategy meeting
SEND health assessments.
Is able to evidence an understanding and ability to
identify and manage attachment disorder/ emotional trauma., impact of neglect
and abuse etc.
Is able to demonstrate an excellent understanding of
mental health and is able to initiate interventions to improve child
resilience and reduce risk of emotional harm and refer appropriately to
CAMHS.
Able to act as a key conduit and contact point between
the child or young person and their Carer/ post 16/leaving care workers where
they have difficulties accessing health services.
Demonstrates the ability to work with carers/residential
units/semi supported living/specialist schools/specialist accommodation
ensuring regular contact with the care settings and ensuring that the setting
us updated on local health services and LPT web based resources.
Able to contribute to court reports
for Care, Placement and Adoption Orders (and equivalent Orders)
Able to confidently manage, provide or
ensure supervision is provided from a health perspective for looked after
children where safeguarding issues arise within the care system
