As a Senior District Nurse, you will also support the wider service line.

A District Nursing Specialist Practitioner Qualification is essential for this post. In addition, you must hold a UK driving licence and have use of a car.

Main duties of the job

Main duties of the job

  • To lead and manage Community Nursing Teams within a locality hub and have overarching responsibility for the assessment, management and evaluation of evidence-based nursing care to patients in a variety of community settings.
  • Line management of the District Nurses / Team Leads who are responsible for day-to-day management of the teams within a locality hub.
  • To promote effective teamwork within the wider primary health care team, delegating activities appropriately, to the benefit of the patient and service delivery.
  • To monitor the quality of care provision and to identify and promote areas for service development.
  • To have overarching responsibility for the performance of the teams and individuals within the hub.
  • To act as a clinical role model and support care delivery for complex patients on the Community Nursing caseloads.
  • To support the Community Service Matron and Clinical Leads in delivering high quality care that delivers best value and meets the requirements of commissioners.
  • To work in partnership with key stakeholders to ensure high quality, integrated and seamless care is delivered.
  • To act as a key point of contact for GPs and other general practice staff at a hub level.

  • Significant post registration experience as a District Nurse
  • Understanding of the service users of the Trust (which could include lived experience of conditions the Trust deals with or of receiving services relevant to those the Trust provides)
  • Highly developed interpersonal skills with the ability to lead nursing teams.
  • Up to date knowledge of the current national and local agendas
  • Evidence of ability to be flexible and adaptable to accommodate a challenging and changing environment.
  • Evidence of ability to lead and motivate multidisciplinary teams, working collaboratively to develop individualised plans of care for the patients on the caseload.

Desirable

  • Non-Medical Prescribing Qualification, or willingness to work towards within the next 18 months.
  • Advanced Communication Skills training.
