Job summary
We are looking for a committed and experienced person to join our wonderful established team of mental health professionals to deliver excellent care within Leicester, Leicestershire & Rutland.
As a Mental Health Facilitator, you will work within primary care to assess and support the mental and physical health care for people who are on the SMI register. This will involve working in collaboration with general practices and primary care colleagues, as well as secondary care services to improve outcomes and peoples quality of life in line with the NHS 10 year implementation plan.
We are committed within LPT to ensure better outcomes for this client group for their mental and physical well-being, as this group are known to have a reduced life span and to be less likely to access physical health support.
You should be experienced and able to work autonomously when required whist still working closely as a team of professionals. Support and supervision will be provided and we promote a positive collaborative leadership style.
For rostered areas, these are set in advance at a team level and you will join an established roster. Your rostered shifts will be confirmed to you when arranging your start date.

Main duties of the job
To work autonomously, taking responsibility for care, quality and clinical activity for a designated caseload of service users
To contribute to care quality performance and clinical activity by providing professional leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality:
Patient safety.
The effectiveness of all health care interventions.
Patient experience.
The environment in which care is delivered.
To participate in the effective clinical leadership of the team.
To actively contribute to the improvement of service and quality of care provision to meet the needs of service users, service users and their families.
To act as role model and participate in the education, development and mentorship of other staff.
To work collaborate with other clinicians and within with wider health, social care and voluntary sector communities as appropriate and promote the role of the Mental Health Practitioner to all stakeholders.
To foster a culture of networking in an open and transparent approach with internal and external partnerships and between all stakeholders.
Responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of service users according to changing healthcare needs and varying levels of complexity.
Our current vacancy is Carillion PCN which covers Bridge Street, Loughborough University, Park View, Pinfold and Woodbrook Medical Centre's.
