Meaningful Activity Co-ordinatorwill share good
practice within a busy, friendly and supportive service that covers 2 physical
wards atEvington centre community Hospital. You will be based on
Beechwood(female) ward but will be expected to cover Clarendon(male) ward if
there is staff shortfall.
You will have support from a Clinical Team Lead,therapy staff and
experienced support staff. Peer supervision and in service training are carried
out on a regular basis and you will have an annual PDR.

Main duties of the job
-To promote patient focused care.
- To organise and manage own time according to a delegated workload to meet the
patients needs.
- To assist the patient in carrying out activities, promoting patient comfort and
wellbeing.
- To contribute to the assessment, planning, implementation and evaluation of
patient care.
- To recognise and report changes in the patients condition, and any issues in
respect of their health and management of care to the registered practitioner.
- To have a good knowledge of infection control, to reduce risk of health care
acquired infections.
- To respond to urgent and emergency situations in accordance with Trust policy and
as a member of the Multi-disciplinary Team.
- To ensure that fully accurate, complete and contemporaneous patient records are
recorded and maintained for each patient in accordance with agreed policy.
- To establish and maintain effective communication with patients and their
relatives/carers.
- To undertake specific organisational and administration duties as required.
- To work with, and support new staff, students and other individuals on placement
to deliver safe and effective care.
- To act in a professional manner and adhere to the LPT Uniform Policy at all times.
- To act as a professional and clinical role model to all staff, patients and carers and
the public at all times.
