Job summary
The purpose of this post is to support the Patient Flow Coordinator with capacity andflow on the general adult and older people's wards.The post holder will support the Patient Flow Coordinator in ascertaining the trustposition regarding bed management resources.They will support in providing a comprehensive handover to the Night Managers andcoordinate receipt of a full handover the following day.The post holder will be responsible for updating and maintaining systems to enableaccurate bed states across the organisation and Greater Manchester.
Main duties of the job
To assist the coordination of admission and discharge activities within adults andolder people acute services.? Ensure an estimated date of discharge is declared within agreed timescales asper operational policy.? Ensure the patients care responsibility lies with PCFT? To participate and assist in establishing an effective system for the managementof beds to ensure efficient patient flow.? Respond to all requests for emergency admission, discussing alternatives withthe Patient Flow Coordinator where appropriate.? Discharges.? To be actively involved in the capture of audit data in response to service need.? To liaise with the Patient Flow Coordinators on a daily basis to inform of thecurrent bad states within the organisation.? To be involved in coordinating the discharge process.? Record all OAPs in line with GM and National guidance.
