Job summary
o To provide support, advice and signposting to the families, friends and carers of people who use the Trust's services.o To support staff to enable families, friends and carers to be recognised as key partners in care.o To promote the Triangle of Care (ToC) standards and principles, and other carer specific initiatives to services and carers.
Main duties of the job
o To act as a point of contact for families, friends and carers of those who use our services by providing emotional and practical support (not for the service user).o To facilitate meaningful carer support groups which provide education and emotional support.o To undertake carers assessment of needo To apply for personal budgets for carers based on their needso To regularly review carers needs and personal budgetso To promote and advocate carers rightso To have an increased knowledge of available carer support networks within Stockportand to act as a carer resource for staff and signpost where appropriateo To collaborate effectively with a range of agencies including PCFT staff, GPs and other health and social care staffo To attend Triangle of Care Meetingso To assist and support the ongoing development and implementation of ToC principles and other relevant carer policies, initiatives & strategies.o To keep and maintain accurate records.o To assist and support in audits and benchmarking best practice processeso To share knowledge and expertise with internal and external peers andcolleagueso To organise and actively participate in relevant meetingso To support the development, delivery and evaluation of training to carers and staff
