Job summary
The post-holder will provide support, based upon assessed needs for patients who are both CPA and Non CPA as per Pennine Care NHS Foundation Trust policy.
You will ensure the highest standard of nursing care provided in accordance with the policies and procedures of Pennine Care NHS Trust.
You will work across hospital and community settings, promoting continuity of care and effective communication. You will participate in the on-going development of the service and in the education programmes of nursing students and other disciplines.
Main duties of the job
An exciting opportunity has arisen for a community support worker role embedded within a busy Community Mental Health Team in Bury. The successful candidate will posses the skills and compassion required to focus on delivering psychosocial interventions with patient with complex mental health and social needs, to support them in their recovery journey. Amongst other duties, the candidate will:
Demonstrate the ability to make a judgement about a patient's mental health or wellbeing needs and act appropriately to manage needs and risks.
Contribute to the provision of a safe clinical environment and promote the dignity of patients.
Support patients whose health and wellbeing is at risk, acting upon and reporting concerns appropriately.
Support patients to engage in meeting educational and vocational needs. This will include arranging initial appointments and attending with the service user if required.
Assist patients to develop skills for everyday living i.e. budgeting money, shopping, cooking, employment, establishing or improving social networks, accessing local services, establishing housing etc.
Support the planning, delivering and evaluation of care programmes with a relevant professional.
Develop and facilitate group work alongside OT, Mental Health Wellbeing Practitioner, nursing and social work staff.
