Job summary

We are seeking a dynamic and suitably qualified and experienced health care professional to support the strategic delivery of the Greater Manchester Child and Adolescent Lead Provider Collaborative (GM CAMHS LPC). Working as part of a wider multi-professional programme team across all providers, to ensure quality responsibilities are met, and to support the collaborative and to be self-assuring and self-regulating.

As an LD Case Manager in Greater Manchester CAMHS LPC the post holder will work as park of a dynamic team in delivering an effective service that oversees all care pathways for young people who require inpatient Mental Health Inpatient care within NHS and Independent sector providers, specializing in the cohort of young people with a Learning Disability and/or autism diagnosis

The post holder will provide case management direction for a caseload of young people and will over see the assessment, intervention and discharge planning throughout their inpatient journey. The role will include robust oversight of the access assessment, whilst ensuring young people are placed in the least restrictive environment with goal orientated treatment

  • Support the LPC in ensuring that services are compliant to all specifications and guidance.
