Job summary
Humphrey House supports people whose mental health problems 18-65 in the community are affecting their ability to carry out their normal everyday activities and hobbies. Humphrey House supports people managing the symptoms of their mental health problems and coping with the changes in life that have been caused by their mental health problems. Humphrey House gives people the opportunity to learn more about mental wellbeing and explore activities and techniques which allow them to maintain their independence and their contacts in the community. Humphrey House offers a range of activities and therapeutic groups which help patients to regain their confidence and manage their symptoms including wellbeing groups and anxiety management. Our aim is for patients to work towards managing their symptoms and regaining their skills and confidence in everyday activities.
This post will work directly with patients to develop skills and interests which will help maintain their recovery following discharge. The successful candidate will help to identify community resources and groups which patients can access following discharge and will introduce the patient to those resources to help make the transition.
The successful candidate will be someone who is confident to go out in the community to make links with community services and find information about activities available to access in the community.
Main duties of the job
The post holder will work as part of Humphrey House CMHT, to undertake allocated duties within the team.Working alongside mental health practitioners, Social Workers, OTs, families and carers in the community and also supporting patients who are on the discharge pathway from the CMHT developing interests and opportunities in the community and ensuring the highest possible standard of care in accordance with the policies and procedures of Pennine Care NHS Foundation Trust.
