An opportunity has arisen for a Community Learning Disability Nurse to join a highly experienced, multi-professional health and social care care team working with people with learning disabilities in Oldham.

The post-holder will be based within the Integrated Learning Disability Team in Oldham.

Oldham Learning Disability team is a truly multidisciplinary team that works with individuals with a learning disability from the age of 18. We deliver high quality health care assessments and interventions to people with complex and challenging needs. The team comprises of community nurses, speech and language therapists, social workers, psychologists, assistant practitioners and psychiatry input. The team is located at Oldham Civic Centre and works across the Oldham areas.

Main duties of the job

Role Summary:

Delivery of evidence based health and behavioural assessments and interventions to adults with learning disabilities who have complex and challenging needs. You will be working in a multi-disciplinary context with the expectation to be person centred, creative and innovative in meeting the health needs of individuals whilst offering support to families and carers.

There will be the opportunity to work across a range of settings and with a range of provider services. There are regular opportunities for continuing professional development and for peer supervision with other Community Nurses working in the Directorate. We are eager to support continual professional development and access to additional training opportunities.

Key Responsibilities of the Post:

The post holder will work in partnership with all members of the Multidisciplinary Team to achieve the 'Good Health' objectives of 'Valuing People'.

Support people with learning disabilities who may exhibit a range of challenging behaviours, which may include verbal and/or physical aggression.

To provide a safe, effective, responsive, efficient, timely, community nursing service to people with learning disabilities, their families and carers.

To undertake a range of assessments to individuals with a learning disability and complex needs. This assessment work requires a specialist knowledge base on which to practice.
