Job summary

This is an exciting opportunity for a Band 6 Mental Health Practitioner to join this dynamic, expanding team. Bury Home treatment is extending their hours to be a 24hr, 7 day a week service.

The post holder will work predominantly nights as part of the HTT team to deliver an effective and efficient response to people who access the service, providing clinical assessments and interventions focusing on relapse prevention and improved quality of life for service users.

Furthermore, will be providing face to face gatekeeping throughout the night, not only for Bury, but across the north division if necessary and review and assess for suitability for HTT any patients referred from a S136 assessment.

We are looking for an enthusiastic, driven registered practitioner who can work autonomously in the community, complete full mental health assessments, manage risk in the community, while also being an integral part of the multidisciplinary team. we also provide clozapine titration in the community.

We are a dedicated team of nurses, OTs, Social workers, QAP and support workers who work closely with our consultant psychiatrist, our colleagues in the community mental health team, acute inpatients wards and the wider members of the MDT

Main duties of the job

The post holder will provide reviews in safe environment where appropriate (include patients who are seeking self-discharge from the wards, providing a safer discharge and any Bury HTT patients that present at A&E).

They will also complete emergency home visits if risk assessed safe to do so, providing a greater flexibility for supervised medication regimes for patients.

As a 24-hr team we will also be able to provide 24 hr telephone support to patients under Bury HTT including patients on the community Clozapine pathway.

To be involved in the ongoing development of the service.

To undertake accurate psychosocial / risk assessments and act as co-worker for service users who are supported by the Home Treatment Team who present with acute mental health problems or psychological distress. To work in close liaison with other members of the multidisciplinary team, including care coordinators, to implement treatment / management plans in order to facilitate Home Treatment.

To undertake the role of the gatekeeper for all people requiring admission to inpatient services, including those requiring assessment under the MHA and to provide an alternative to hospital admission through Home Treatment interventions where appropriate.

It is expected that you will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act and community care legislation.
