Clozapine and Physical Health Clinic Deputy

Job summary

The post holder will assist in the coordination and delivery of the Clozapine clinic and a physical health strategy for locality service users including the assessment of physical health through a clinic setting and within an individual's own home environment where needed. This will include regular monitoring of physical health measures, healthy lifestyle advice and to ensure access to physical health interventions anddeliver them where appropriate to do so.

The post holder will be an integral member of the Clozapine clinic team and will deputisefor the clinic lead and be a senior on site presence.You will liaise with GP's and other health professionals where indicated to deliver holistic care and a complete client health record to ensure aquality and integrated service is delivered.

Main duties of the job

  • To ensure the delivery of the service within the Clozapine clinic is in accordance with CPFT Clozapine operational policies.
  • To ensure the delivery of the service within the Physical Health clinic is in line with CPFT Medicines Management policies.
  • To attend agreed meetings, meet supervisor and team manager on matters of service development.
