Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To provide comprehensive, holistic therapy assessment and rehabilitation to patients within their home environment, maximising independence within a 4 -week pathway.
To hold clinical responsibility for patients within the service and Therapy Practitioners within their team. To prioritise workload including that of the Therapy Assistants within the team.
To work proactively, in collaboration with the wider multidisciplinary team to ensure patient flow and safety.
To assist in the development of the Integrated Care Workers and Therapy Practitioners to support the clinical delivery of the service.
