Job summary
We are seeking an enthusiastic Band 5 Physiotherapist to join the Integrated Stroke Early Supported Discharge (ESD) and Community Neuro Rehabilitation Service. The Integrated service provided both stroke rehabilitation and community neuro rehabilitation supporting service users with a variety of complex needs.
The ESD Service is a key pathway for stroke survivors allowing them to leave hospital earlier than they may have otherwise; they receive up to 6 weeks of intensive rehab in their home/community or in one of our inpatient Community wards in Cambridge or Peterborough. Community Neuro Rehabilitation work with stroke survivors post ESD input or with greater needs in addition to a wide variety of neurological conditions.
This is an exciting opportunity to join an established multi-disciplinary team comprising of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab assistants.
This role requires the successful candidate to manage a clinical caseload across the South of Cambridgeshire. Their base will be either Huntingdon or Cambridge dependent on location nearest to home.
The role involves visiting patients in their homes, on wards or community gyms. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. You will provide comprehensive assessments and deliver evidence-based specialist neuro physiotherapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.
