12 Month Fixed-Term Contract
We have a fantastic opportunity for a Dietitian keen to progress their career in diabetes. Our community diabetes team is well established and provides high level virtual, face to face and group support for patients with Type 1 and Type 2 diabetes.
This 12-month maternity cover post offers excellent development opportunities with possible scope to support diabetes education including DESMOND. Good knowledge and experience in diabetes is required and DESMOND training is an advantage. The service is spread around Cambridgeshire and the principal base for the role can be negotiated (Peterborough, Huntingdon, Fens, Cambridge).
The post holder needs to be HCPC registered and a car driver.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. - To deliver a specialist diabetes role for the Nutrition and Dietetic Service in Cambridge & Peterborough NHS Foundation Trust (CPFT)
- To work as a dietetic practitioner within uni-professional and multi-professional team environments with regular supervision
- To assess and treat a case load of patients in line with the model and process for dietetic consultations and maintain associated records
- Deliver a high standard of dietetic care in the community
- To deliver a large number of education and training sessions related to diabetes, such as DESMOND, DAFNE and PDAC
- To supervise or monitor the work of other dietetic staff, assistants and students

- Provide a dietetic service to specialist Diabetes primary care clinics based throughout the. To work with a wide variety of complex Diabetes both Type 1 and Type 2 adult clients. To take into account such factors as the clients presenting condition, past medical history, social factors, anthropometry and biochemistry to nutritionally diagnose, give appropriate advice and formulate treatment plans in consultation with patients, relatives and carers.

- To consult diabetes clients aged 16 and above as a sole practitioner or jointly eg with a DSN, either in their own homes or in community settings.
- To work with the band 7 in the development and maintenance of clinical standards, guidelines, pathways and evidence based practice in diabetes.
- To support the Team Leads in development and delivery of new ways of working to increase efficiency and effectiveness of the service, while continually monitoring and evaluating progress.
- To liaise with wider MDT and external providers to improve patient access to Psychological Therapies (IAPT) and attain optimal patient outcome.
- Advise on the safe and appropriate use of prescribed nutritional supplements for patients taking account of the influence they have on their d
- To develop and update written resources, for example diet sheets and patient advice leaflets.
- Act as a point of reference on the management and treatment of patients with diabetes and gastroenterology conditions for non specialist colleagues and other health professionals, giving advice and supporting their clinical practice.
- Provide training and updates about nutrition in diabetes and related issues to a wide range of audiences including colleagues, catering staff, health and social care staff, carers and clients.
- Provide a pre-agreed amount of dietetic services outside of core working hours which may include an evening service.
- Maintain accurate records of all patient consultations and related work carried out at each clinical session, using the Nutrition and Dietetic Process and Model for consultations.
- Complete Templates on Systmone as required to satisfy the expectations of the teams clinical standards.
