Job summary
We have an exciting opportunity for a rehabilitation focused senior Occupational Therapist to join our Intermediate Care Service based at the Princess of Wales Hospital, Ely.
The successful applicant will work predominantly within inpatient rehabilitation wards as part of an in-reach model of therapy. They will also have the flexibility to work across pathways within the intermediate care community team, home first pathway. Applicants will be offering holistic clinical assessment, treatment, discharge planning and specialist occupational therapy knowledge. Successful applicants will play a vital role in supporting patient flow through the pathway as well as providing clinical skills and knowledge to the MDT.
This position offers flexibility, variety, and the opportunity to be involved in the transformation of our intermediate care services.This is a new operating model of flexibility, merging our community and in-patient therapy staffing to improve patient outcomes with a seamless therapy pathway. Main duties of the job
- To work across Pathway 1 (care at home) and Pathway 2 (Health Interim beds and Inpatient Rehabilitation units) for the Intermediate Care service.
- To work as an autonomous practitioner within the integrated multi-professional team.
- To be reasonable for patients on the shared caseloads of community and/or inpatient settings.
- To undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
- To identify patient needs, agree goals and provide appropriate therapy interventions, provide equipment and rehabilitation programmes.
- To take a holistic approach to all therapeutic interventions, working across professional boundaries at times.
- To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
- To provide support and education to peers, junior staff, non-registered staff, and students.
