Job summary

A rare and exciting opportunity has arisen for a mental health Band 3 support worker to join the Older People's Crisis Resolution Home treatment team based in Cambridge. The successful candidate will be working within the Dementia Intensive Support Pathway within CRHTT-OP (Older People's Crisis Resolution Home treatment team) working across the Cambridge area.

The CRHTT-OP/DIST key objectives are to focus on home treatment for patients and their carers living with dementia. This includes the management of risk, prevention of hospital admissions and working with carers to prevent carer burnout and care placement breakdown.

The successful candidate will bring not only experience of supporting people living with dementia (this is essential) but an ability to think 'outside of the box', be imaginative, innovative and highly motivated to make a difference. Main duties of the job

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. You will be supporting the team's Community Psychiatric Nurses/ Mental Health practitioners by attending Initial assessments for patients who are experiencing Behavioural and psychological symptoms of dementia. Patients will be seen in care home environments as well as in their own homes.

You will work closely with carers and family and help identify what will help the person and their carers to live well with dementia. You will work closely with care coordinators as well as working independently offering regular intensive care support, identifying triggers to expressed behaviours, role modelling, and psycho education. You will also be responsible for carrying out relevant physical health observations.
