We are looking to recruit a Registered Mental Health Practitioner who has experience of working as part of an MDT and understands System working with internal and external partners.

Main duties of the job
In your role as the Bed flow manager, you will work closely with the Out of Area Team (OATS) manager and local crisis teams, you will be expected to lead on supporting the sourcing out of area beds, be able to link with private providers, work with the referring team(s) to understand the clinical needs to ensure the right bed is sourced.
This role includes regular weekend/out of hours working including early/ late shifts depending on service needs.

The post holder will be responsible for the reviewing and supporting CPFT patients that are currently placed in an Out of Area bed.
The post holder will work independently, using their own initiative/ judgement. This role is central to supporting the flow of patients managed outside of the trust and to support repatriation/discharge where needed.
The post holder will support discussions and decisions regarding transfers between wards and to community services as well as to facilitate the sourcing of contract and spot purchase beds when a CPFT bed is unavailable.
The post holder will demonstrate excellent communication skills, be able to co-ordinate and collate real time information on current and predicted bed availability and facilitate the effective use of acute hospital and community wide bed capacity.

- Registered AHP
