New Hire Bonus - £1500

The permanent posts are eligible for a £1500 New Hire Bonus. An initial payment of £500 will be paid in the first 1 month and a final payment of £1000 will be paid 1 year after your start date. This will be pro-rata for part-time posts. Internal applicants are not eligible.

CPFT are delighted to offer you the opportunity to be part of the OPAC Crisis Team, supporting Older People aged 65 and over or with the diagnosis of Dementia.

Our aim is to provide multi skilled team support to families and Older People in Crisis by delivering short-term, recovery-focused interventions. We are looking for practitioners who are passionate, dynamic, and skilled and ready for a new challenge to improve our patient's experiences. The Team will have experienced clinical leaders working alongside support staff to deliver planned episodes of care and ensure people have access to appropriate support.

Main duties of the job

Ideally you will have experience working with complex family issues and be able to demonstrate our Trust values (Professionalism, Respect, Innovation, Diversity and Empowerment) in delivering care and working as part of a team.

Main duties include:

To deputise for the Team Leader across the full range of managementresponsibilities, including the performance review of junior staff. To take on the role of care coordinator for clients and provide leadership support to others, to ensure that full assessment of individual care needs are undertaken, that care is planned, implemented and reviewed on a regular basis. To act as role model and promote effective teamwork across the service to meet individual client needs. To provide a crisis and home treatment service to patients, which incorporates dementia intensive support. To ensure that the CPA and Mental Health Act is adhered to at all times, and to work within the boundaries set out by the Trust's Policies and Procedures.

1. The post holder will provide a good role model, develop and maintain good systems of communication, be able to communicate with a wide range of people in a range of situations. 2. The post holder will monitor and maintain the health safety and security of self and others. 4. Assessment, treatment, intervention, planning, implementation and evaluation. Ensures that legislation, policy and procedure are considered when planning specific aspects of service developments and projects. Monitors and evaluates outcomes of specificaspects of service development and projects.
