Job summary

Applications are invited for Clinical Fellow posts in Neurocritical Care (Intensive Care) at The National Hospital for Neurology & Neurosurgery, Queen Square, London, for a 6-month period, that can be extended to a year or more, subject to satisfactory appraisal. We have three posts available that will commence 5th February 2025. Interviews will take place 19th November 2024. The successful applicants will work under consultant supervision at a level best matched to their learning needs and experience.

Main duties of the job

The posts are also suitable for those wanting to further their experience of neurocritical care for specialist training and are also suitable for CESR equivalence in neurocritical care and higher general intensive care medicine.

Job description

Job responsibilities

The National Hospital for Neurology and Neurosurgery provides neurological and neurosurgical services on a regional, national and international basis. The neuroscience critical care unit is a 28 bedded unit with approximately 850-950 admissions a year of which 35-40% are planned elective admissions. We provide care for neurology, acute stroke and neurosurgical patients, in one of the largest dedicated neurocritical care facilities in the UK. There is excellent senior support 24/7 for the unit, with two daily ward rounds by ICU consultants and a dedicated anaesthetic registrar to help with airway management, line placement and training, and other out-of-hours intensive care issues. There are opportunities to be involved with teaching, MDT simulation sessions, clinical governance and quality improvement projects to strengthen portfolios for applications for training posts, as well as developing technical skills and competencies in Intensive Care Medicine. The successful applicant will join a team of 17 doctors, including HEE trainees from various specialities (ICM, IMT and neurosurgery), Clinical Fellows (ST1+), CESR ICM fellows and senior neuroanaesthesia fellows (ST5+) as part of an EWTD compliant full shift rota. This post is a remunerated as a locally employed doctor in line with the 2016 terms and conditions of service.

There are weekly MDT neurocritical care meetings to discuss patients with neurologists, neurosurgeons, clinical psychologists and AHP. The monthly M&M meetings include interesting learning cases where decision making, and ethical dilemmas are discussed. There is a formal weekly educational programme, consisting of consultant/senior delivered teaching, journal club, as well as informal bedside teaching, tea trolley teaching, in situ MDT simulation and a CVC simulation course for the junior clinical fellows. Some of the consultants are POCUS accredited and can mentor FUSIC heart, lung and transcranial ultrasound. You will be allocated an Educational Supervisor to enable regular appraisal and support in the development of a learning plan and portfolio.

The successful candidate will Share the responsibility for the assessment, resuscitation and treatment of all patients referred to the Critical Care Unit, working as part of a team with the on-call ICU consultant. Work alongside our trainees on a full shift rota. This is a full shift rota and the successful candidate will be required to be resident in the hospital during the working hours allocated to them on the rota Share the responsibility for direct clinical supervision and teaching of the trainee staff, nurses and medical students. Assist in the provision and development of a safe, effective and efficient critical care service and support the Trust in its aim to provide the best possible care to patients. Carry out all duties at the highest possible standard taking responsibility for individual patients in their care. Comply with Trust Health and Safety policies and employ the basic principles of infection control practice to maintain a safe and healthy environment for patients, visitors and staff. Maintain good electronic clinical records and effective working relationships with medical, nursing and allied healthcare professionals on the critical care team. Take an active role in the departmental clinical governance activityApplicants must be eligible for full registration with the GMC at time of appointment and hold a current licence to practise. Post-graduate examination qualifications are desirable, but by no means essential. This post is not recognised for training by the Postgraduate Dean, Faculty of Intensive Care Medicine or by the Royal College of Anaesthetists, although the unit itself is.
