Job summary

An exciting opportunity has arisen for suitably qualified and experienced nurse to join our Integrated Care Operations Team. We are seeking a highly committed and motivated individual who will ensure that we are identifying all suitable patients in UCLH acute beds for our Virtual Ward beds and other external care pathways.

The successful applicant will work closely with staff across all disciplines between hospital and community to ensure the early identification of suitable UCLH@Home and Virtual Ward patients and support the safe, timely and effective transfer of the patients from the acute beds.

  • Registered Nurse Part 1, 2 or relevant section of the register
  • Teaching course
  • Relevant clinical course

Experience
