Job summary

The post holder will be required to provide effective and timely care within a multidisciplinary team setting for the Older People Community Mental Health Team (OPCMHT) who present with both organic and functional mental health problems, their carers and families and others who are significant to the individual.

Main duties of the job

The post holder to demonstrate expert practice in the care of clients with mental health needs and maintain clinical expertise. taking responsibility for initiating risk assessment, mental capacity, best interest assessments, crisis planning, dialog plus and core assessment, ensuring the care and interventions provided within the ISS are evidence based and in accordance with NICE guidance.

To provide mental health training to the Trust and external agencies, taking responsibility for planning delivering, implementation and evaluation of training.

To have a good working knowledge of community resources for older adults with mental health needs and their carers to access, to signpost, provide advice, education and support.

Be responsible and accountable for the management of own caseload as directed by the OPCMHT manager.

To be highly skilled in working with potential aggressive behaviour of clients in the community.

To develop care plans in collaboration with the patient, carer, and other relevant person/agency.

Maintain contact with service users on their caseload during periods of admission, including attendance at MDT meetings. To take lead in the planning and implementation of the clients re-integration back into the community.

Perform thorough physical health assessments, encompassing monitoring of vital signs, weight, blood pressure, blood sugar and other relevant indicators to ensure comprehensive health evaluation.
