The role is a joint role between BEH & a Barnet Primary Care Network (PCN).
The role of Mental Health Practitioner is to work closely with clinicians in Primary Care Networks to assess and treat clients with significant mental illness.
This key interface role between Primary Care and Secondary Mental Health Services will provide assessment and interventions for clients presenting with a wide range of mental health needs.
The Mental Health Practitioner will provide dedicated support and presence within primary care settings, to both work with clients and offer advice for primary care colleagues to plan appropriate and timely Interventions.
These evidence-based therapeutic interventions will include psycho-education, brief solution focused therapy, behavioural activation, as well as reviewing and monitoring medication, in line with NICE Guidance and in-liaison with the Locality Consultant Psychiatrist.
This role will require developing and maintaining an effective interface with Clinicians in secondary Mental Health Services, including the team Locality Consultant Psychiatrist and Psychological Therapy Pathway leads.
A key part of the role is to build long-term relationships and improve communications with Primary Care colleagues, Voluntary Care Sector colleagues and Community Mental Health Services.
Main duties of the job
o Undertake high quality assessments of people who are identified as having a significant mental illness that may require treatment, interventions and potentially a referral to secondary mental health services.o Providing effective care, treatment and interventions to clients on a defined caseload within the Primary Care Network.o Undertake review and monitoring medication, in liaison with the Consultant Psychiatrist.o To facilitate timely referrals into Secondary Mental Health services, IAPT, Wellbeing Network, and other local community services.o Engage with clients in primary care who for various reasons do not engage with Secondary mental health services.o To ensure that statutory and legal requirements are incorporated safely within each service users care plan.o To undertake regular and thorough risk assessment for each service user you support.o To be able to work autonomously within your Primary Care Network and identify and report risk in a timely manner using systems in place..Professional Developmento Opportunity to work using different models, including Trauma Informed Care and Medically un-explained symptoms.
