Band 4 Referrals Co-Ordinator

Job summary

The Referrals coordinator will provide administrative support to the service. They will work within service level standards to deliver an efficient and effective service; dealing with telephone and personal enquiries from patients, carers and members of the public, GP's and other healthcare professionals.

Main duties of the job

  • Opening of internal and external referrals. Checking the referral has been correctly completed with all relevant information.
  • Signposting referrals to the appropriate services.
  • Send the referral through to the Screening team.
  • Attend the Multidisciplinary Team meeting to accurately record the outcome of the discussion.
  • Monitor the onward plan for the referral.
  • Update and monitor the tracking of the referral on the screening and initial assessment pathway.
  • Manage the referral in-box, actioning all requests sent through.
  • Take personal responsibility for all tasks involved in the role.

  • Good knowledge and proficiency of Trust systems - eProcurement/Clares/eRostering to include RiO.
  • Excellent verbal and written communication
  • Keyboard skills to ensure accuracy, speed, and optimum use of functions
  • Maintains confidentiality and observes Information Governance and Data Protection rules
  • Work supportively within a team
