Job summary
In line with the NHS Long Term Plan and the Community Mental Health Framework laid out by NHS England the Trust is taking part in a large transformation programme across North Central London (NCL) looking to integrate the care delivered for mental health problems more closely based around place. The vision will be to have 'no wrong door' for people presenting with problems, to have shorter more purposeful interventions within secondary care and deliver far more services in a collaborative way.
Main duties of the job
The Mental Health Advanced Clinical Practitioner (MH ACP) will use highly specialist knowledge and skills to provide healthcare autonomously to deliver integrated mental health and physical health to persons who identify with having a mental illness and may otherwise fall between the gaps. The role will work with people both with an established mental health diagnosis as well as those without a diagnosis, not under a mental health team, presenting with a deteriorationin mental state not warrantingcrisis team, linkwork or secondary care intervention but who would benefit from some short-term support. The role will see patients in a PCN assigned base, all GP practices are part of designated PCN's so the role will see patients whose GP surgeries are within that PCN. This will be for initial assessment and the opportunity to offer a brief intervention for their mental health issue. If the person needs further input for their mental health issue, the MH ACP will refer as appropriate.
As the role is working on behalf of a PCN the roles require flexibility with a strong collaborative approach and will work equitably on behalf of all GP surgeries within, the PCN boundaries.

This role is a joint role between BEH & a Barnet Primary Care Network (PCN)
The PCN will ensure that the MH ACP completes an induction into their PCN and Primary Care. The induction timetable will support developing knowledge of primary care and the wider supporting workforce roles within the PCN. An induction into the mental health Services and Pathway will also take place in collaboration with the PCN and Barnet Mental Health Core Teams.
The MH ACP role is accountable for independent clinical assessment, diagnosis and treatment of people identifying with a Mental Health need. In addition to this, the MH ACP is also responsible for appropriately referring/hand over toalternative/relevant specialties services where appropriate for any necessary investigations.
The MH ACP role will practice at an advanced level demonstrating in depth knowledge and competence in aspects of mental health needs; encompassing enhanced assessment, critical thinking, intervention planning and delivery and clinical management skills that are evidence based. The MH ACP role will deliver high-quality person-centered and trauma informed care.
The MH ACP role will link with the new Mental health Community Adult Core Teams and will lead in the creation of robust communication and referral pathways, being a conduit for primary care PCNs and Barnet Community Mental Health Teams.
The MH ACP role will work closely with other community offers, social prescribers, co-developing person-centered plans with our colleagues in social care, voluntary services, and housing, enabling people to receive the support they require.

Essential
- Current registration with NMC, HCPC and or other statutory body
- Specialist Practitioner
- MSc or PGdip in Advanced Clinical Practice or equivalency criteria as per national standards -
- Independent non-medical prescriber (V300) within scope of practice
Desirable
- Relevant Professional health degree. If PGdip must have previously completed an MSc
- Evidence of recent CPD
