Job summary

The post holder will assist in providing a high quality, high performing mental health and physicalhealth care for complex service users within HMP Wormwood Scrubs inpatient unit that mirrorsservice provision in our other establishments.. The post holder will work alongside clinical & disciplinestaff to provide individualized assessment, care/management plans and risk assessments for thepatients on the unit. You will be a confident practitioner that thrives in working in a fast paced oftenhigh pressured environment.The successful candidate will provide mental health assessments, brief interventions, advice,treatment physical care and therapeutic activity to men within the inpatient unit. The post-holder willbe a resource to staff across healthcare and the wider prison providing advice and support to thecare of these service users in order to ensure best outcomes.

Main duties of the job

To work as a member of the H3 Healthcare unit at HMP Wormwood Scrubs.Be responsible and accountable for the management of own caseload as directed by the Modern Matron.To undertake assessments of patients referred to the inpatient unit by primary care staff, officers, probation, substance misuse services and other referral sources on the wings.To provide interventions, advice and care for patients who may have both physical and psychological health problems.Jointly facilitate group work and therapeutic activity with multi-disciplinary colleagues (including education staff).To complete detailed clinical risk assessments at first and subsequent assessment and admission and accurately record and report key risks.To develop care plans in collaboration with the patient, carer and other relevant person or agency.To review and evaluate treatment plans at agreed intervals in collaboration with the patient and members of the multidisciplinary team.To provide primary nurse function to the service users on your caseload.concerned with mental health.To influence the development of high standards of nursing care, ensuring that appropriate systems are in place in order to support evidence based practice.To be conversant with all guidance and operational procedures in relation to offender healthcare at HMP Wormwood Scrubs.To participate in the ACCT process within the establishment, ensuring agreed healthcare interventions required from the Team are provided.
