Medicine Optimisation and Accredited Checking Pharmacy Technician

Job summary
The post-holder will work as part of the Pharmacy Team, facilitating medicines optimisation systems by:
- undertaking medicines reconciliation for newly admitted patients;
- assessing patients own drugs for their use on our wards;
- supporting the coordination of in-patient medicines supply;
- facilitating the patient discharge process and undertaking patient counselling.
You will support clinical governance processes by:
- undertaking pharmacy and ward/ team-based audits;
- providing education & training to other healthcare staff, trainees and students;
You will also strengthen our Trust dispensary team by being an accredited checking technician and participating in our weekend/ Bank holidays rota.
You will have excellent written and verbal communication skills, and time management skills and be able to work at all the Trust Pharmacy Services sites.
There will be an opportunity to develop through e.g. education, training, and mentoring schemes.
The post holder will be: GPHC registered; have a Level 3 NVQ Pharmacy Service or Level 3BTEC diploma in Pharmaceutical sciences or equivalent; and will also be an accredited checking pharmacy technician.

Main duties of the job
The post-holder will:
- Support the Lead Pharmacist in delivering the medicines optimisation agenda.
- Visit wards to undertake medicines reconciliation (review) process for new admissions.
- Provide advice on medicines to patients and carers,
- Assess patients' own medicines for use during the inpatient stay,
- Transcribe non-stock medicines items,
- Supervise trainees and students on the wards/units and within the dispensary,
- Promote clinical governance through ward/team-based clinical audits,
- Accurately label and dispense medication against all types of prescriptions and requisitions including controlled drugs, outpatient clinics and inpatients and TTAs,
- Act as one of the departmental accredited checking technicians of clinically screened dispensed items.
- Aid the coordination of workflow in the dispensary, liaising with the Dispensary Manager.
- Manage pharmacy dispensary and stores in the absence of the relevant operational managers, as needed.
- Work with the Distribution and Procurement team to maintain an effective supply chain, when needed.
- Participate in the ward top-up service and issue stock drugs to wards and departments on requests.
