Our friendly and supportive team, works Mon-Fri 8.30am-4.30pm and is based at Chase Farm Hospital, Enfield.
Enfield Memory Service is an accredited service with MSNAP (Memory Services National Accreditation Programme) and is committed to on-going service and staff development
EMS has developed a dynamic model of assessment where the clinical team carry out comprehensive assessments, supported by a 'floating consultant' and develop care/treatment plans to offer a range of interventions . You will manage your own caseload, carry out specialist assessments and provide a person-centred approach, including support forfamily carers.
You can expect to join a supportive team, committed to service development and providing a high standard of memory assessment and person centred care.
Enfield Memory Service is an innovative service and you will have the opportunity to contribute to the on-going development of the service.
The service is clinic based, however, on occasions the clinical team are required to assess people in their home environment.
Main duties of the job
- To actively manage an allocated caseload of people referred to the service, ensuring a patient centred approach to care
- To carry out robust specialist assessments for patents referred to the service and feedback to the reviewing consultant.
- To determine clinical priorities and plan patient care accordingly, based on thorough risk assessment and risk management plans.
- To carry out Triage phone calls to people newly referred to the service
- To work with the CST coordinators to help plan, facilitate, and evaluate therapeutic groups at the EMS
- Provide patients and their carer/s with a service that is culturally sensitive and gives due consideration to individual needs.
- To contribute to and lead on aspects of continuous improvement initiatives including audits, quality assurance and satisfaction surveys.
- To carry out robust specialist assessments for patients referred to the service and feedback to the reviewing consultant.
- To be competent using a variety of cognitive assessment tools, including the ACE 111 and RUDAS
- To be responsible for raising urgent clinical concerns with the team leader/senior colleagues/consultant and referring to other services as appropriate.

EDUCATION AND QUALIFICATIONS
Essential
- Professional registration (Occupational Therapist and registered with the HCPC or Registered Mental Health Nurse - RMN and registred with the NMC.
- Evidence of ongoing CPD in their portfolio
Desirable
- Evidence of further study, particularly in the field of Dementia care, or older people's mental health

EXPERIENCE AND KNOWLEDGE
