Job summary
We have an exiting vacancy for an experienced RNLD/RMN to work as a Senior Practitioner within the Choices Learning Disabilities Partnership(CLDP), a community forensic learning disabilities team in the NCL area.
The CLDP covers a diverse population within the Boroughs of Barnet, Enfield, Haringey, Camden and Islington. It is a service that is passionate about reducing health inequalities and improving the mental and physical health of the service users in it's care.
The CLDP provides continuous monitoring and risk assessment to ensure the safety of service users and the general public
Main duties of the job
The successful applicant will be enthusiastic about improving the health experiences and outcomes of people with learning disabilities. This will involve coordinating care for people with a forensic history and liaising with external agencies. The successful applicant will implement care pathways and follow the guidance of the S117 Aftercare.
The successful post holder will be an experienced Registered Nurse in Learning disabilities and/or Mental health with extensive experience of working in the community.
The post holder will provide clinical and leadership expertise and work closely with the Team Lead and all relevant stakeholders to deliver key outcomes for our service users.
About us
The Choices Learning Disabilities Partnership is part of the Forensic Learning Disabilities Pathway, provided by the North London Forensic Service (NLFS). The Forensic LD Pathway consists of Medium and Low Secure wards and a Community Team. The post holder will interact with the multi-disciplinary teams across the pathway. The NLFS is part of the newly formed partnership between Barnet, Enfield & Haringey Mental Health NHS Trust and Camden and Islington Foundation Trust, now known as the North London Mental Health Partnership. The NLFS is an outstanding service providing healthcare services to people living in the North Central London region.
