Job summary
We are looking to appoint a qualified and experienced Community Mental Health Nurse to the post based in the Barnet Adolescent Service.
We are a multidisciplinary team offering an outpatient CAMH service to 13-17 year olds meeting specific referral criteria, from across the borough of Barnet. We are currently based at the Holly Oak Clinic in Edgware Community Hospital.
We offer our service to those adolescents referred to CAMHS who would benefit from a more focused and immediate response in order to encourage engagement in those who may well find this difficult, e.g. those with chronic self-harm or newly emerging psychosis, high risk adolescents, such as those in the criminal justice system or long term social worker, dual diagnosis with substance misuse, homeless or no longer living with their parents but not looked after, or have left school and not as yet established in further education or training.
We do not operate a waiting list and see adolescents within 2-4 weeks or sooner if there is high risk. We work collaboratively as a team sharing cases in order to be able to support young people up to 3 times a week. We also use the CYP IAPT model where appropriate and regularly use their outcome measures.
Main duties of the job
To support the specialist adolescent pathway in Barnet CAMHS with the systematic provision of a nursing service within the adolescent outreach team in CAMHS in Barnet for young people and their families. This is a community based nursing post located within the specialist Barnet Adolescent Team (BAS) offering outreach work to families with specific emphasis on adolescents with acute and complex mental health problems and who present with high risk to themselves and/or others. The post holder will be an active member of the integrated team providing intensive outreach and support to families and individuals in order to act as an alternative to hospital admission.
To act as a resource and support to the generic CAMHS teams, the Adolescent Service (BAS) in Barnet and the Service for Children and Adolescents with Neurodevelopmental disorders (SCAN) and the Paediatric ward at Barnet Hospital. The post holder would be expected to establish good working relationships with local GPs as well as education providers, local authority children's services and voluntary sector organisations in order to better meet the needs of young people and their families.
The post holder will directly contribute to the positive working environment within the multi-disciplinary team. The post holder will require a high degree of professional autonomy and self-management, and will be required to account for their professional practice through appraisal and audit.
