An exciting opportunity has arisen within Haringey Learning Disabilities Partnership.
We are looking for an enthusiastic and skilled Learning Disabilities Nurse to join the Assessment & Intervention Team (AIT).
AIT provides support to adults eligible to receive services from Haringey Learning Disability Partnership who are at risk of placement breakdown in the community and/or admission to psychiatric hospital due to deterioration in mental health and/or challenging behaviours, as well as providing support and intervention for psychiatric hospital discharge (Transforming Care Programme) to reduce risk of readmission. AIT provides intense person-centred support and intervention for the person with a learning disability and their family and/or carers with home visits, monitoring calls and behavioural analysis, using individualised easy read information and other resources to improve engagement, and understanding. AIT consists of 2 Senior Nurse Practitioners (Band 6) and 2 Assistant Psychologists (Band 4) who co-work cases along with other professionals within HLDP's multi-disciplinary team. Regular supervision is provided by Consultant Psychiatrists, Clinical Psychologists and Team Leader and Specialist Practitioner (Mental Health Pathway).
Main duties of the job
The post holder will:
- Be competent in decision making, assessment and the management of clinical needs.
- Provide training on issues relating to the behavioural support, mental health and physical health needs of people with learning disabilities
- To hold a caseload as an autonomous practitioner.
- COMMUNICATION
- To accurately record all contacts with service users in case notes on Mosaic in line with local record keeping policies.
- To ensure effective communication with service users, relatives and carers, advocates, staff and others.
- To ensure that all communication takes place in a manner that is consistent with legislation, policies and procedures.
- QUALITY
- To participate in the development, maintenance and monitoring of service standards, collect and collate data / information effectively for the purpose of audit, research and service performance.
- To identify, implement and evaluate, with others, areas for potential service improvement and agrees further action.
- RISK ASSESSMENT
- To understand the range of risks potentially faced by service users.
- To work in partnership with others to identify and assess the nature, location and seriousness of the particular risks.
- PROFESSIONAL/PERSONAL DEVELOPMENT
- To receive clinical supervision and participate in appraisal and performance development.
- To adhere to the NMC code of professional conduct.
